The first one I need is actually very very simple, here we go:
1. I need to know how many icons you want me to do.
Temi> I need two icons.
2. If you have 5 images you want included in your icons, then just post
the url of the 5 images. If you dont, then just leave that section out.
Temi> Just one image, this:

3. Tell me the text you need for each icon.
Temi> Just the word: Boss Cart
4. Tell me the size of the icon you want. (125x125) or (100x100)
Temi> I would like 80 by 80 pixels ad 125x125 pixels
Graphic type: Icons (ie banner, icons etc) :
Size: 80x80 and 125 x125 pxls
